Sec. 103.002. FORM AND CONTENTS OF APPLICATION

An application for a late ballot must:
(1) be in the form of an affidavit; and
(2) include, in addition to the information required by the applicable provisions of Section 84.002, the date of death of the decedent and a statement of the relationship of the voter to the decedent.
