Sec. 603.252. LICENSE APPLICATION

Latest version.

(a) An applicant for a perfusionist license must submit a sworn application accompanied by the application fee.
(b) The executive commissioner shall prescribe the application form and by rule may establish dates by which applications and fees must be received.
